- สำเนาทะเบียนบ้าน 1 ชุด
- สำเนาวุฒิการศึกษา 1 ชุด
- สำเนาหลักฐานการเกณฑ์ทหาร 1 ชุด
สิทธิประโยชน์ที่จะได้รับ
เงินเดือนต่อรองได้, เงินรายได้ค่าบริการไม่น้อยกว่า 3,500.- บาท/เดือน, กองทุนสำรองเลี้ยงชีพ, ประกันภัยอุบัติเหตุและสุขภาพแบบกลุ่ม, ห้องพยาบาลภายในวิทยาลัย (โดยแพทย์จากโรงพยาบาลศิริราช), สวัสดิการพนักงานวิทยาลัย, อาหารพนักงาน 2 มื้อ, เครื่องแบบพนักงาน และสิทธิในการสมัครเป็นสมาชิกสหกรณ์ออมทรัพย์ ม. มหิดล
